01. Create a new work sized 340x340. Now make a 280x280 circle selection with the Elliptical Marquee Tool (M). Center this selection on the work.
Make a new layer and Edit > Fill (Shift+F5) the selection with color #CCCCCC. Create a smaller selection sized 40x40 center of the shape and press Delete leaving shape as shown.

02. I have turned off the background visibility so we can work better on the transparency of the disc.
Now change the blending opacity to 25% and click the Apply a layer style button to apply Bevel and Emboss

04. Now you should get the base of the disc as shown.

05. Now create a new layer (Layer 2) on top of the shape.

06. Using the same technique, make a circle selection sized 270x270 and center it on the disc shape. Fill the shape with #CCCCCC.
Now make another selection sized 90x90 and center it on the disc then press Delete to get the shape as shown.

07. Now pick the Gradient Tool (G) and choose the Angle Gradient option. Now click on the gradient preview area (the area labelled CLICK HERE in the image shown below).
The Gradient Editor will pop up. Use settings as shown. Then click the Randomize button until you get a nice gradient with the colour that suit your disc data surface.

08. After you've selected the gradient, make a new layer (Layer 3) and set the opacity to 50%. Layer > Create Clipping Layer (Ctrl+G) to clip it with Layer 2.

09. Now fill this layer (Layer 3) with the selected gradient. Stroke as shown. Make sure you fill from the center of the circle. You should get the gradient fill as shown in the image.
It is suggested that you try with a different gradient and try it with the 50% opacity. When you are statisfied with this step, proceed to next.

10. Now Layer > Duplicate Layer the gradient layer.

11. Because the shine of the data surface is always mirrored, we are going to flip the gradient fill to achieve the mirrored effect.
Edit > Free Transform (Ctrl+T), then right click the transformation area and select Rotate 180 degrees from the menu. Press Enter to apply transformation.

12. Now set the original layer to 100% opacity because we are going to merge the duplicated copy to this layer. Now click on the duplicated layer (50% opacity) and Layer > Merge Down to this layer.

13. Finally, set the single gradient layer to 50% opacity again.

14. Make a selection as shown. Hold Shift while making a selection with the Elliptical Marquee Tool (M) to make perfect circles. First make a selection slightly larger than the border between the disc data surface and the disc plastic.
Hold Alt to substract selection when you make a new selection. After you get that done, make a new layer and Edit > Fill (Shift+F5) it with color #333333

15. Set the layer to settings as shown.

16. You should get something as shown. Now click on the data surface layer (Layer 2) and Select > Load Selection.
With the selection, make a layer at the bottom of all the disc layers. Fill the shape with black #000000

17. Now Filter > Blur > Gaussian Blur with the following settings.

18. Set the layer opacity to a desired value. Here I have set it to 50%. You can move the shadow few pixels down by having the Marquee Tool (M) selected and tapping the Down arrow while holding Ctrl.

19. Your disc should look like this now.

20. Now for the final touch, select the Pen Tool (P) and make a path on the circle shown. It is recommended for you to work with grid enabled.
When you're done with the path, select the Type Tool (T) and click on the path and start typing. Adjust the text color and size to your own liking.
Please note that Text on a path is a new feature only available in Photoshop CS
